If you are a homeowner in Breathitt, Knott, Leslie, or Perry County, KY and need help with affordable home repairs, please complete the form below. This program serves income-eligible homeowners, including 2022 or 2025 flood-affected households.
Submitting this form allows our team to begin a detailed review of your situation and determine which home repair or rehabilitation funds may be available to you.
What happens after you submit this form?
After we receive your intake form, a member of our Home Repair team will contact you to confirm next steps. You will be mailed a document checklist outlining the information we need to continue reviewing your eligibility.

Bring the documents to our office at 2871 North Main Street in Hazard (across the street from The Mother Goose), or bring them with you to a scheduled meeting with our Home Repair staff.
Once all required documents are received, our team will review your information and determine eligibility based on available funding and program guidelines. We will contact you to explain your status and, if approved, discuss next steps and potential timelines.
